Chevra Kadisha and Connections Group invite you to learn more about health care directives—a one hour zoom workshop led by retired hospice social workers, Ann Gonski and Susan Sugarman. Two opportunities to participate: Friday, May 20th at noon and Sunday, May 22nd at 3:00 pm.

Whether or not you have completed a Health Care Directive, we will support you with
suggestions for having the necessary conversations with those who might need to act
on your behalf.

Who might benefit from this workshop?

If you are a younger adult and do not have a health care directive, this workshop may help you to determine who will be the best choice to serve as your decision maker in case you are under anesthesia or incapacitated by an accident or illness.

If you already have a directive, this is an opportunity to review your choice of decision-maker and the instructions to the agent so that the document reflects your personal wishes and values.

If you want to learn more about different types of directives, we will explain the difference between an Advance Directive for Health Care, a POLST, a Power of Attorney for Finances, and a Dementia Directive. We will also provide links to sites that provide options for on-line directives.

For those who wish to attend in-person to discuss, draft, or amend their health care directives, we will facilitate an in-person workshop at Beth El on Saturday afternoon, June 4.
